Forms & Surveys — Collect Data from Your Website Visitors

Introduction

Answe forms appear within the conversation flow, making data collection feel natural rather than like filling out a boring form. Attach forms to any answer option to collect names, emails, phone numbers, feedback, or any custom data.

How It Works

Creating a Form

  1. Create a question and add an answer option
  2. Click the option and choose "Add Form"
  3. Use the form designer to add fields:
    • Text — Single-line text input
    • Email — Email with validation
    • Phone — Phone number with validation
    • Textarea — Multi-line text for longer responses
    • Dropdown — Select from predefined choices
    • Checkbox — Yes/no toggle

Form Templates

Start faster with pre-built form templates:

  • Contact Form — Name, email, message
  • Newsletter Signup — Email and preferences
  • Feedback Form — Rating and comments

Viewing Responses

All form submissions appear in your Chat Inbox with the full conversation context. You can:

  • See each field value
  • Track when it was submitted
  • Reply to the submitter
  • Export data

Key Benefits

  • Conversational — Forms appear as part of a conversation, not a standalone page
  • Validation — Built-in email and phone validation
  • Multiple field types — Text, email, phone, textarea, dropdown, checkbox
  • Templates — Pre-built forms for common use cases
  • Conditional flows — Show different follow-ups based on form answers
  • Analytics — See which fields are most/least filled out
  • No page reload — Smooth inline submission
